New bridge number request

• Local system
• Trunk highway system - use this form only when an Interstate, US or MN trunk highway is on or under the bridge

Notification to add, remove, or rehab a structure; or a partial structure over traffic

• Add structure - send when the bridge is open to traffic
• Partial structure - send when the bridge is spanning over traffic but is not completed.
• Remove structure - send when the bridge is no longer in service
• Rehab or Overlay structure - send when the rehab or overlay is completed
